Introduction      

 

Leeds Mind is the lead agency in a partnership with Hamara, Touchstone, Barca-Leeds,

Better Leeds Communities and Leeds Irish Health and Homes which has the contract to provide the Connect for Health Social Prescribing Service for Leeds South East (LSE).

 

Main purpose of the post

 

The post holder will jointly manage the team of 13 Wellbeing Coordinators with the full-time Team Leader. Wellbeing Coordinators are the front-line staff who provide assessments and signposting support to the individuals who use this service.  The successful candidate will also play a lead role in outcome and quality performance and will provide a link to the GP surgeries

 

Main tasks and responsibilities:

 

Staff management

 

  1. Performance manage and support the Wellbeing Coordinators to achieve individual, team and service targets; ensure that all staff are enabled to deliver a high-quality service and receive appropriate support for their own development and wellbeing needs.
  2. Develop strong working relationships with all key partners to ensure consistent performance management of the wellbeing coordinators.
  3. Conduct 1:1’s with the Wellbeing coordinators in conjunction with the partners ensuring that relevant policies and procedures are followed.

 

Service Management

 

  1. Support the Connect for Health Service Manager to provide leadership and direction for the service in terms of development and being flexible and responsive to change.
  2. In the absence of the Service Manager, to deputise for them.
  3. Support and contribute to the monitoring, reporting and evaluation of the Connect for Health service.

 

Service promotion & Networking

 

  1. Liaise with stakeholders & key partners to maintain referrals sources. Ensure that effective communication between GP Practices and the service is maintained and feed back to the CCG as required.
  2. Promote the service by maintaining excellent links with GP practices, partners, statutory and non-statutory services across LSE.
  3. Create and attend meetings and opportunities to network with clients, partners and other stakeholders in LSE on a regular basis.

 

Training & Development

 

  1. Participate in 1:1, performance management and development, including Leeds Mind compulsory training programme.
  2. Undertake any staff development and training which is required, to enable the job to be performed in the best possible way.
  3. To engage with CPD (Continuous Professional Development) in line with identified targets and objectives (individual and service).
  4. Where appropriate attend external clinical supervision as arranged by the line manager.

 

Organisation

 

  1. Attend and participate in service/team meetings.
  2. Ensure service compliance with Leeds Mind and the relevant partner agency policies and practices.
  3. Work within Leeds Mind Health and Safety Policies to ensure a safe and healthy working environment for staff, volunteers and students.
  4. Promote the values, ethos and philosophy of Leeds Mind.
  5. Promote the possibilities for empowerment of service users.
  6. Be involved with wider organisation activities.
  7. Undertake any other reasonable duties as and when required.
